What Is Degenerative Disc Disease?

Despite the word “disease,” degenerative disc disease (DDD) is actually a normal part of spinal aging rather than a true disease process. As we age, spinal discs lose water content and become thinner and less flexible.

Discs are mostly water in youth, up to 80% in young adults. By middle age, significant water loss occurs. This dehydration makes discs less effective as shock absorbers and more prone to damage.

The term “disease” is somewhat misleading. Many people have degenerative changes on imaging but no symptoms at all.

How Degenerative Disc Disease Develops

What Is a Herniated Disc?

A herniated disc occurs when the tough outer layer of a disc (annulus fibrosus) develops a crack or tear, allowing the soft inner material (nucleus pulposus) to push through.

This can happen suddenly from an acute injury like lifting with poor form, or gradually from repetitive stress. The herniated material can compress nearby nerves, creating significant pain and neurological symptoms.

How Herniations Happen

Acute herniation can result from sudden twisting, heavy lifting, or trauma. The disc material ruptures through the outer layer during a single incident.

Degenerative disc disease develops gradually over years or decades. You might not know when it started. Herniated discs often have a more identifiable onset, sometimes traced to a specific incident or activity.

Type of Pain

DDD typically causes chronic, aching back pain. Herniations often cause acute, sharp pain that may include radiating nerve symptoms.

Neurological Symptoms

Radiating pain, numbness, tingling, or weakness in an arm or leg strongly suggests nerve compression from a disc herniation. DDD alone doesn’t typically cause these neurological symptoms unless it’s created significant stenosis.

Age of Onset

DDD typically becomes symptomatic in middle age or later. Herniated discs can affect younger people, often occurring in the 30s and 40s.

Imaging Findings

MRI distinguishes between the two. DDD shows disc height loss, dark disc signal (indicating dehydration), and end plate changes. Herniations show disc material extending beyond the disc space, sometimes contacting or compressing nerve structures.

Can You Have Both at the Same Time?

Absolutely. In fact, it’s common. Degenerated discs are actually more prone to herniation because their weakened structure is less able to contain the inner nucleus under load.
